Terms and Conditions of The Heart of Diabetes Campaign
Introduction: Type 2 diabetes dramatically increases a person's risk for heart disease and stroke and often is associated with other cardiovascular risk factors, such as high blood pressure, cholesterol disorders, obesity and insulin resistance. Unfortunately, most people with type 2 diabetes are not aware of these prevalent health risks.
The Heart of Diabetes program was created by the American Heart Association to not only raise awareness of this issue, but help those with diabetes take action to lower their risk for heart disease and stroke.
The Heart of Diabetes has launched a new campaign to seek out stories about type 2 diabetes. Individuals with type 2 diabetes, or individuals who know someone with type 2 diabetes, are invited to submit inspirational stories of their struggle and success with type 2 diabetes [hereinafter "My Story"].
Submission: Applicants can submit their stories via e-mail, via the web application and via mail. In all cases, the applicant must use the standard form created for sharing their story, found on IKnowDiabetes.org.

Terms and Conditions: By submitting My Story, I agree to be bound by the following terms and conditions, and I acknowledge the following:
- I or the person whose story I am submitting is at least 18 years of age and a legal resident the United States, its territories and possessions.
- I have been diagnosed by a healthcare provider licensed to practice medicine in the United States as having type 2 diabetes. If requested, I am willing to provide to the American Heart Association a written letter providing proof of my medical condition as is related to type 2 diabetes.
- All statements, remarks, personal identifying details and facts and all other information of any kind or nature which I make, give or relate in connection with My Story will be personal to me and no other person and will be true, correct and accurate. Also, I will not omit or withhold information in connection with My Story for the purpose of misleading anyone or misrepresenting any facts or circumstances.
- By submitting My Story, I understand and agree that the American Heart Association, its attorneys, representatives and agents, including the AHA’s advertising and promotion agencies, may use My Story for any purpose, including but not limited to, reproduction, disclosure, editing, marketing, distribution, communication, creation of derivative work, and promotion, for editorial, commercial, promotional and all other purposes, including posting on the American Heart Association's websites.
- I give permission to the American Heart Association to use the personally identifiable information I provide in accordance with its Privacy Policy.
- I also understand that the American Heart Association is under no obligation to use the information I submit.
- I am enrolling in The Heart of Diabetes program, or other program ("AHA programs"), a program for people with diabetes created by the American Heart Association, Inc. ("AHA").
- I am engaging in the AHA Program voluntarily and for my own personal reasons. I understand that it is my responsibility to consult with a physician regarding diabetes and heart disease.
- The AHA programs I may enroll in, now or in the future, may advocate or involve physical activity, including exercise, which may involve certain risks. By participating in AHA programs, I assume all associated risks associated with physical activities. It is my responsibility to consult with a physician to determine my ability to engage in any and all activities associated with the AHA Program. It is also my responsibility to use equipment, clothing, and technique that are appropriate for the activities related to the AHA Program. I am solely responsible for my own safety.
- I agree to not sue, and to release, indemnify and hold harmless, The Heart of Diabetes, the AHA, its affiliates, officers, directors, volunteers and employees, and all sponsors of the AHA programs sponsors and the agents of such sponsors, from any and all liability, claims, demands, and causes of action whatsoever, arising out of my participation in the AHA program, whether arising from the negligence of any of the above parties or from any other cause. The foregoing release, indemnification, and hold harmless shall be as broad and inclusive as is permitted by the state in which I live.
- I consent to the aggregation of my non-identifying information with like information from other people, and I consent to the release of such aggregated information to other parties, including but not limited to the sponsors of AHA program. I authorize AHA Program and the AHA to mail me information about the AHA Programs or about other AHA offerings.
- I acknowledge and agree that the AHA may discontinue certain AHA programs without notice to me and that I shall have no continuing rights in the AHA programs upon such termination.
- I assert that I am the person about whom the information I am providing relates.
If any portion of this agreement is held invalid, the balance shall continue in full force and effect.
